How to apply online

Choose the right service

Prepare scans of originals
Official service pages note that a simple screenshot of a Diia document does not replace the required scan of an original for this procedure.
Complete and sign the application
The application is submitted through the official Resident Portal, which guides you through signing and status tracking.
Collect the card
Official pages list the Passenger Service Centre for personalized-card collection: 10 Horbachevskoho St, Lviv.

Call the LeoCard information line at 0-800-303-007 or check the current rules on the official website. LvivPulse does not accept documents and has no access to your LeoCard profile.
